What is Support at Home?
Support at Home is the Australian Government’s program for older people who want to keep living independently at home. It replaced the old Home Care Packages (HCP) on 1 November 2025. The Commonwealth Home Support Programme (CHSP) continues as its own entry-level program — the government has announced CHSP will continue until at least 2029 — and your assessor may refer you there if your needs are simpler. After a free assessment through My Aged Care, you receive a quarterly budget for the services you need — and you choose a registered provider to deliver them.

That choice matters. Your provider decides who walks through your door, how reliably they come, and how far your budget goes. Rosewood Gardens is family owned, we roster a consistent small team wherever possible, and clinical care plans are overseen by an appropriately qualified clinician — with nursing oversight where the person's needs and service type require it.

How funding works

Assessment
Register with My Aged Care (we’ll help). An assessor visits your home and determines your support level and budget.
Care plan
We sit with you and your family, listen to what matters, and design a plan that spends your budget where it counts.
Ongoing support
Your matched carers begin. We review the plan with you regularly and adjust as your needs change.
Who pays what?
Government funding covers clinical supports (nursing, allied health) without a participant contribution. Contributions can apply to independence and everyday-living services and depend on the person's circumstances. From 1 October 2026, approved personal care has a 0% participant contribution — services must be in the support plan and available within the person's budget. We give you a clear written breakdown before anything starts, and if contributions would cause hardship, we help you apply for relief through Services Australia.
Short-term pathways
Support at Home is not only ongoing care — targeted short-term programs help you recover, adjust or prepare for change.
Assistive technology & home modifications
Equipment and home changes that keep you safe and independent — rails, ramps, mobility aids, shower chairs and personal alarms — funded through the separate AT-HM stream, on top of your quarterly budget. We guide you through what your assessed needs cover.
Restorative care
A short, focused program to rebuild strength, confidence and independence — often after illness, a fall or a hospital stay.
End-of-life support
Compassionate palliative support at home, coordinated with your medical team, so final months are lived with comfort and dignity — surrounded by family.

Thinking about changing providers? We make it safe and simple
You have choice over your providers and can change, subject to your service agreement and plan arrangements. Many of our clients came to us this way.
1 · Review your current agreement
Check your service agreement and notice period — we'll help you read it if you like. If you change providers, available funds are reconciled and transferred under the program rules; we can help you understand the steps before you switch.
2 · Meet us — free and confidential
We confirm we can provide the supports you need, show you exactly what they cost at our published rates, and answer every question. No obligation, no Rosewood Gardens joining fee.
3 · We coordinate the transition
With your consent, we coordinate records, schedules and key contacts with your current provider — working to minimise any disruption to your support.
4 · Start with a consistent team
We agree your service agreement and commencement date together, then match you with a small, consistent support team wherever possible — backed by a registered nurse.
