Getting started, step by step
1 · Tell us about your goals
Call, enquire online or ask your support coordinator to contact us — tell us the supports you're looking for.
2 · We review together
We look at your NDIS plan with you and confirm our service capability, areas and availability.
3 · Meet your proposed team
Where practicable, we introduce the proposed team before regular supports begin — the right fit matters.
4 · Agree and begin
Service agreement, pricing and commencement date agreed in writing. Complex and high-intensity supports may need additional clinical information, risk assessment and worker training before services begin.
Who we support
We support adult NDIS participants with physical, intellectual and psychosocial disability, including people with complex and high-intensity needs. Whether you live in your own home, with family, or in supported accommodation, our job is the same: understand your goals, then build reliable support around them.
- Agency-managed, plan-managed and self-managed participants welcome
- Culturally sensitive support — many of our team are bilingual

How we work with you
Expert care planning. We start with your NDIS plan and your goals — not a standard roster — and design supports that actually move you toward them.
Caregiver matching. Personality, language, culture and interests matter. We match support workers you'll be comfortable with, and keep them consistent.
Seamless coordination. One point of contact for you, your family, your support coordinator and your plan manager. Reports and communication that keep everyone aligned.
Goal achievement. We review progress with you and adjust — because a plan that doesn't change with you isn't a plan.

NDIS Registered Provider
We meet the NDIS Practice Standards and our registration is independently audited. Support coordinators and families are welcome to ask for our registration details and capability statement — or verify us on the NDIS Commission's public provider register.
Safe, skilled support you can rely on
Words are easy in this sector — independent audits aren't.
Independently audited
Rosewood Gardens is a registered NDIS provider. Our registration is maintained through the Commission’s independent quality audits against the NDIS Practice Standards; our current registration groups and audit scope are available on request.
NDIS Practice Standards
Registered provider obligations, worker screening, incident management and behaviour support rules — met and independently audited, not self-declared.
Clinical governance
A registered nurse stands behind every high-intensity support plan, with oversight, training and competency checks for every worker we send.
Thinking about changing providers? We make it safe and simple
You have choice over your providers and can change, subject to your service agreement and plan arrangements. Many of our clients came to us this way.
1 · Review your current agreement
Check your service agreement and notice period — we'll help you read it if you like. Choice and control means your supports can move within those arrangements.
2 · Meet us — free and confidential
We confirm we can provide the supports you need, show you exactly what they cost at our published rates, and answer every question. No obligation, no Rosewood Gardens joining fee.
3 · We coordinate the transition
With your consent, we coordinate records, schedules and key contacts with your current provider — working to minimise any disruption to your support.
4 · Start with a consistent team
We agree your service agreement and commencement date together, then match you with a small, consistent support team wherever possible — backed by a registered nurse.

Let’s talk about your NDIS support
Send the form and a dedicated Care Manager will call you back — usually within one business day. Or talk to a real person now.
From your home to our home
Care at home for as long as possible — and if 24/7 care ever becomes necessary, a carefully planned pathway to our Melbourne residence with the same people and relationships throughout. That's something many providers simply cannot offer.
